I formatted the disk and installed Mac Mojave and Office. 2.Please check if your HD Graphics driver is the latest version. If not, please update it. It's a Mac so the drivers are installed and managed by the OS.

I'm not sure if formatting Mac will remove all folder mentioned. If it's convenient for you, you check if there are duplicated items or folders in the same local disk. Moreover, I would suggest you try to ensure it is not caused.
